Q. Trapezius Botox, isn't it only
for when you have shoulder pain?
A. No. It reduces enlarged trapezius muscles
to make the neck look longer and create a right-angled shoulder line
It is used far more often for cosmetic purposes.
Q. How long does the effect last?
A. It varies from person to person, but usually lasts 4 to 6 months,
and with repeated treatments, the muscle becomes thinner,
so the maintenance period tends to get longer.
"Reducing enlarged trapezius muscles
makes the neck look longer
and creates a delicate right-angled shoulder line,
so it's an essential procedure before wedding shoots"
— Director Wi Young-jin (Hongdae Beautystone Clinic)

First, what does it mean
when the trapezius
'gets thicker'?
To be honest,
among people searching for trapezius Botox,
those who come in saying 'because my shoulders are severely stiff,' and
those who come in saying 'my neck looks short and my shoulder line looks uneven,'
the two groups are split at almost similar rates.
What's interesting is,
the cause of both concerns actually
starts from the same place.
The trapezius (trapezius muscle) is
a very broad muscle that runs from the back of the neck to the shoulders and upper back.
When this muscle becomes developed or stays tense,
two things happen at once.
First, as the curve between the shoulders and neck rises,
it creates the visual effect of a shorter neck.
Second, because the muscle itself is knotted up,
chronic shoulder stiffness and headaches often come along with it.
Here's the key insight.
Trapezius Botox is not simply
"an injection that lowers the shoulders."
When Botox temporarily blocks the command signals
going to the muscle, the force of muscle contraction decreases,
and as a result, the muscle volume itself
starts to decrease.
As this decrease in volume gently lowers the line
between the neck and shoulders, the neck appears longer,
and the line leading to the shoulder tip
is tidied up to look close to a right angle.
That's why many people get it 3 to 4 months before
wearing a wedding dress or off-shoulder outfit.
Not just for shoulder pain,
but to change the silhouette itself.
👨⚕️ Director Wi Young-jin's Key Summary
The real effect of trapezius Botox
is not just pain relief.
As the enlarged muscle decreases,
the neck looks longer than it actually is,
and the shoulder edge line is organized to look close to a right angle,
creating a delicate silhouette.
These two things happening at the same time
is the core of this procedure.
So how much do I need
— Approach by dosage and type
What makes this a bit ambiguous is that,
there isn't a fixed answer to the question,
"How many units of trapezius Botox do I need?"
The approach changes depending on muscle development, shoulder width, whether there is pain,
and the purpose.
If I divide patients into broad categories during consultations, it looks like this.
① Those whose trapezius is highly developed, and
whose main goal is line correction
We usually start at 50 to 100 units per side.
If it's your first time, it's better to start with a smaller amount and see the response.
"Putting in a lot at once makes it wear off faster" is not true.
② Those with shoulder stiffness and headaches
Since the upper trapezius tension is often the cause,
we approach it by focusing on controlling the upper part of the muscle.
The focus is more on the upper area than on line correction.
③ Those preparing for a wedding or event
Because it takes about 2 to 4 weeks for the effect to appear,
we recommend getting it at least 6 to 8 weeks before the event.
If you come right before the shoot, you won't see enough effect
before the day passes.
But there's one important thing here.
It's something many people misunderstand,
Trapezius Botox is not a fat-reduction procedure.
Because it atrophies the muscle,
it's more accurate to say it becomes "thinner" rather than "loses weight."
So it may not look as dramatically
different as you expected.
Especially with just one treatment.
However, after 2 to 3 repeated sessions,
the muscle itself tends to become thinner,
so the results last longer and feel more noticeable.
And there's one more thing I should definitely mention:
the trapezius is also a muscle involved when lifting your arms
or carrying heavy objects.
If the dose is too high, you may feel like your shoulders have lost strength,
or that lifting heavy objects feels awkward.
Still, these are temporary symptoms,
and they usually recover naturally within 1 to 2 months.
It's not to the point where daily life becomes truly inconvenient.
However, if you use your shoulder muscles a lot,
I always mention this point.
Frequently Asked Questions
Q1. Does trapezius Botox cause resistance?
A. Resistance to Botox itself is rare.
However, not keeping the intervals too short between repeated treatments,
and not unnecessarily increasing the dose,
is what leads to better long-term results.
Q2. Can I go about daily life right after the procedure?
A. Yes, you can.
The procedure itself takes around 10 minutes at the shortest
and doesn't require separate anesthesia.
However, on the day of the procedure,
it's best to avoid rubbing the treated area hard,
strenuous exercise, and saunas.
Q3. Can it be done together with facial Botox?
A. In most cases, both procedures can be done on the same day.
If you let us know in advance which areas you'd like treated together during the consultation,
we'll make a plan accordingly.
Please be sure to consult thoroughly before the procedure
and make your decision.
