To cut to the chase, Sculptra is the product among the three collagen boosters that creates the greatest volume in the long term. PLLA, the Strongest Collagen-Stimulating Ingredient

The main component of Sculptra is PLLA (Poly-L-Lactic Acid)* microparticles. It is a safe, biodegradable polymer that has been used for sutures for decades, and it acts as a powerful signal to trigger collagen production as its particles slowly break down.

PLLA* (Poly-L-Lactic Acid): A synthetic polymer belonging to the alpha-hydroxy acid family. It is a proven ingredient used for a long time in sutures, bone screws, and facial implants. It slowly biodegrades within the skin over 18 to 24 months, continuously stimulating collagen synthesis. The difference in this degradation speed and particle size is why Sculptra creates the most significant volume among all collagen boosters.

The biggest difference compared to Radiesse (CaHA) is that its immediate volumizing effect is subtle. PLLA particles themselves are small in volume, and because the carrier is absorbable, you won't see a dramatic change right after the session. You might even feel, "I just got treated, but my face looks exactly the same in the mirror." However, as collagen begins to build up properly from 1 to 3 months onwards, it delivers a fuller volume than Radiesse.

In the Comprehensive Review of PDLLA for Aesthetic Procedures, the collagen-stimulating intensity of PLLA and PDLLA is reported to be higher than that of CaHA. However, because PLLA degrades more slowly and has larger particles than PDLLA, it offers the strongest stimulation but also carries a relatively higher risk of nodule formation.

An Unexpected Origin Story: HIV Patients

Sculptra’s first clinical application was not cosmetic. In August 3, 2004, the US FDA first approved its indication for treating facial lipoatrophy in patients with HIV.

Here is the background. With the introduction of anti-HIV medications — HAART (Highly Active Antiretroviral Therapy)* — in the late 1990s, the life expectancy of HIV patients increased significantly. However, as a side effect of the drugs, lipodystrophy began to occur, causing facial fat to shrink from the temples and below the cheekbones. This physical change heavily impacted patients' quality of life, adding social stigma on top of the physical changes.

HAART* (Highly Active Antiretroviral Therapy): A standard treatment combination using multiple anti-HIV drugs. Though it successfully restored patients' life expectancy close to normal, lipodystrophy, which causes loss of fat in the face and limbs, was reported as a side effect.

Sculptra was the first FDA-approved treatment to restore facial fat loss in these patients. Clinical trials followed, showing that restoring hollow areas under the cheekbones and on the temples significantly improved patients' depression and anxiety scores. This efficacy is well-documented in various publications, including clinical studies on deep subcutaneous application of PLLA in HIV patients.

Later in 2009, this same approval was expanded for general cosmetic use (cosmetic facial lipoatrophy and wrinkles), and today, it is used worldwide—including in Korea and Europe—as a standard collagen booster. In other words, Sculptra has a unique history of beginning out of medical necessity and expanding into cosmetics.

Target Treatment Areas: Best for Broad Volume Recovery

Conversely, areas to treat with caution:

  • Patients wanting to avoid puffiness — Since it creates the most distinct volume, other options are better if you prefer a very conservative change.

  • Patients whose top priority is avoiding nodules — Given the larger particle size, nodules can be felt if they form. The injector’s expertise and depth of placement are critical here.

Treatment Protocol: One Full Cycle of 3 Sessions, 4 Weeks Apart

Sculptra is not a one-time treatment. The standard protocol calls for receiving 3 sessions at 4-week intervals to complete a single cycle. As the stimulation accumulates over three treatments, collagen synthesis occurs consistently and reliably.

Right after the session, the area will appear slightly swollen as the hydration carrier is absorbed, which then subsides. At the 1-month mark, you may feel as though nothing has changed, but the actual volume begins to shift around months 2–3, reaching peak results by month 6. The duration of Sculptra is over 24 months, the longest among all collagen boosters.

Among the cases that Dr. Wi Young-jin and Dr. Kim Ga-eul reviewed together at the symposium, patient satisfaction at 6 and 12 months was consistently highest in those who completed all 3 sessions. Some stop after just one treatment thinking, "I don't think it's working," so we make sure to advise patients in advance that the key to Sculptra lies in completing the cumulative cycle.

Frequently Asked Questions

Q. I don't see any results 1 month after my session. Did it fail?

This is a normal response for Sculptra. It is the product with the most subtle immediate results, and true changes start to appear after 2 to 3 months. Only when you complete the 3-session cycle at 4-week intervals will the cumulative results build up. We recommend evaluating the final outcome around month 6 rather than judging after the first session.

Q. If this treatment started out for HIV patients, is it safe for general cosmetic patients?

Yes, it is perfectly safe. The medical formulation and injection methods are the same; only the indication was expanded for aesthetic use. In fact, because its safety was first proven in the highly demanding context of treating immunodeficient patients, it has exceptionally abundant safety data. That said, it is a treatment where the injector's skill—such as controlling injection depth and volume—greatly dictates the final outcome.

Q. Which is better, Radiesse or Sculptra?

It is not a matter of which is superior, but rather their timelines are different. If immediate results are your number one priority, go with Radiesse; if having the maximum volume after 6 months to 2 years is your priority, Sculptra is your best choice. Very often, we use both differently depending on the area on the same patient — Sculptra for the cheeks (maximum volume) and Radiesse for the jawline (immediate definition).
