That's why recognizing the warning signs within 30 minutes to 4 hours post-procedure is the real game-changer for a full recovery. If caught in time, it can be completely reversed with hyaluronase.

In short: If the treated area or its surroundings turn a dark blue-purple, or if you experience throbbing pain, sudden paleness, or blistering within 30 minutes to 4 hours after your treatment, please contact the clinic immediately. This is different from normal bruising. Recovery is highly achievable with hyaluronase within 4 to 6 hours.

 

Normal vs. Warning Signs — Hour-by-Hour Checklist

Right after a nose filler treatment, it is completely normal to experience slight swelling, redness, and a minor bruise. That is the standard healing pattern. However, warning signs show up in different shapes at different times. Here is an easy-to-read summary:

The key differences lie in the 'shape of the discoloration' and 'whether it gets better or worse over time.' Normal bruising gradually fades, whereas an occlusion remains unchanged or darkens further.

During & Immediately After — Throbbing Pain and Paleness

The most critical signs usually appear during the procedure itself. Throbbing pain (pain that beats in sync with your heartbeat), paleness (the treated area suddenly losing its color), and blanching* (color not returning after being pressed) — if any of these three occur, your injector should immediately recognize it and stop the procedure.

* Blanching: A phenomenon where the skin remains white and does not instantly return to its normal pinkish tone after being gently pressed. This is a primary sign that capillary blood flow in the area has stopped, acting as the earliest emergency indicator of a vascular occlusion during a procedure.

Keep an eye out for these same signs for 30 minutes following your treatment. Look closely in a mirror to check the color of the treated area plus 1–2 cm surrounding it. If the normal skin tone progresses from pale → blue-purple → black, it is an emergency. A regular bruise is typically dark red with soft, blurry borders. An occlusion, on the other hand, has distinct borders and a lace-like (mottled) pattern.

* Vascular Occlusion: A state where blood flow is blocked because filler has entered or is compressing a blood vessel. The entire area supplied by that vessel is cut off from oxygen and nutrients → if not treated within 4 to 6 hours, it can progress to tissue necrosis.

30 Mins ~ 4 Hours — Distinguishing Bruises from Occlusion

This window of time is the golden hour for self-evaluation. Even if your clinic is closed, checking three key elements—color, pain, and sensation—can help you distinguish an occlusion from a typical bruise.

  • Color — Bruises are dark red; occlusions are a dusky blue-purple. Bruises present as a solid patch, whereas occlusions show a lace-like pattern or a V-shaped distribution (following the path of blood vessels).

  • Pain — Bruises hurt mainly when pressed; occlusions cause an ongoing, throbbing pain even when left untouched.

  • Sensation — Normal areas feel slightly tender or mildly numb; an occlusion feels cold to the touch and may be accompanied by numbness.

The clinical difference between a bruise and an occlusion lies in whether these three symptoms appear together. Having just one symptom usually means it's a normal bruise, but experiencing all three points strongly toward an occlusion.

24+ Hours — Don't Wait if You See Blistering

After 24 hours, normal bruises will begin to fade. An occlusion will do just the opposite. The appearance of blisters or pustules indicates that epidermal damage has already begun, and black discoloration means the damage has reached the deeper dermis.

Even at this stage, please reach out to your treating clinic immediately. If addressed within 48 hours, partial or full recovery is still highly possible using hyaluronase and supportive therapies. Beyond 48 hours, necrosis progresses, and the treatment plan shifts toward tissue reconstruction.

Caution If you experience blurry vision or see dark spots, go to an emergency room immediately. In glabellar (between the brows) filler injections, if the supratrochlear artery becomes occluded, the filler can flow backward into the ophthalmic artery and block the retinal artery, posing a serious risk of blindness.

If You Spot a Warning Sign — Contact the Clinic Immediately

If you detect any warning signs, please call your clinic immediately—regardless of whether it is nighttime or the weekend. Waiting for the next business day is the most common reason for unnecessary scarring. If you cannot reach your clinic, go to the nearest emergency room and explain that you suspect a 'vascular occlusion* following a dermal filler procedure.' Emergency room staff are well-trained in the protocol for administering hyaluronase.

The clinical progression of nose filler complications is highly time-sensitive. Please do not hesitate to take action if you notice these signs. Coming in within the first 4 to 6 hours gives you the absolute best chance for a complete and smooth recovery.

If you experience abnormal color, pain, and skin temperature all at once, you should suspect an occlusion. Instead of waiting, contact your clinic. Sending over a quick, clear photo is often enough for a primary assessment.

Q. My clinic is closed because it's nighttime. What should I do?

Most clinics that perform nose filler procedures provide an after-hours emergency contact number. If you weren't given one, please seek out another medical aesthetic clinic that offers late-night services or visit a nearby emergency room. Mentioning that you suspect 'vascular occlusion after filler injection' will guide the medical staff to initiate the appropriate hyaluronase protocol.

Q. Am I completely safe if I don't have any warning signs?
