While minor nose filler side effects like scarring, slight shape asymmetry, or swelling can resolve over time, a truly critical issue is vascular occlusion*. If a key blood vessel is blocked, the entire area supplied by that branch is cut off from oxygen and nutrients → leading to necrosis (tissue death) within a few days. Around the glabella (between the eyebrows), backflow can even travel to the ophthalmic artery, potentially causing blindness. Let's look closely at why the nose is more vulnerable than other areas, and which parts carry the highest risk.

In short, the nose has one of the most complex networks of branching blood vessels in the face. When an occlusion occurs in the glabella or nose tip, there is very little collateral circulation (alternative blood pathways), meaning even a small blockage can directly lead to necrosis. Your top priority should be understanding the relatively safer zones (nasal ala, side profile of the nose) versus the high-risk zones (glabella, nose tip) before scheduling your treatment.

 

Why Nose Blood Vessels Carry Higher Risks Than Other Areas

Three main arteries branch out around the nose: the angular artery, dorsal nasal artery, and supratrochlear artery. Because all three are quite thin with numerous branches, the risk of a filler accidentally entering or compressing them is significantly higher than in other facial regions.

What makes it even more critical is that these branches directly connect to the artery leading to the eyes (ophthalmic artery). The supratrochlear artery runs up from the glabella to the forehead, where it meets the ophthalmic artery. The dorsal nasal artery at the tip of the nose also originates from a branch of the ophthalmic artery. Consequently, if filler backs up during a glabellar treatment, it can block the retinal artery, and there have been reported cases resulting in blindness.

* Vascular Occlusion: A condition where filler enters a blood vessel or compresses it, blocking blood flow. The entire area supplied by that vascular branch is deprived of oxygen and nutrients → if not treated within 4 to 6 hours, it will progress to necrosis.

How Occlusion Occurs — Two Mechanisms During Injection

Occlusion typically happens in one of two ways:

  • Direct Injection (Intra-arterial): When the needle enters the artery and injects filler directly inside, blocking the entire vessel instantly.

  • External Compression (Extrinsic compression): When too much filler is injected near an artery, and the resulting external pressure pinches the vessel shut, progressively stopping blood flow.

Direct injection is more sudden, widespread, and dangerous. This is why experienced practitioners use safety protocols such as a cannula (a blunt-tipped needle), aspiration (pulling back gently on the plunger before injecting to check for blood), and injecting small amounts in gradual stages.

The Timeline from Occlusion to Necrosis

Tissue does not die immediately when blood flow is blocked. There is a predictable timeline, and this window of time is the golden hour that determines the chances of recovery.
