Ulthera + Thermage Swelling: When Does It Go Down? Tips to Speed Up Recovery

This is a question I hear quite often — from our coordinators and during consultations alike.
Knowing when the swelling goes down before you schedule your appointment
matters more than you might think!
(*Note: this post focuses on Ulthera swelling,
not Thermage swelling.)

Ultherapy Prime produced less swelling than 1st-generation Ultherapy (Legacy).

1. Why Does Swelling Occur After Ulthera + Thermage?
Ulthera + Thermage (Ulthermage) is a portmanteau of Ulthera and Thermage —
RF (Radiofrequency) + HIFU (High-Intensity Focused Ultrasound) —
a lifting treatment that delivers heat energy separately to the deep SMAS layer and the dermis.
*The SMAS layer sits between the muscle and fat.
*The dermis is the skin layer just beneath the surface.
Think of it that way — easy to remember!
The treatment works by stimulating collagen regeneration deep within the skin.
During this process, a temporary inflammatory response occurs in the surrounding tissue, causing swelling and warmth.
This is a completely normal reaction that indicates the treatment is working as intended.
(*Even when the treatment goes well, swelling may not appear at all!)

2. How Long Does Ulthera + Thermage Swelling Actually Last?
It varies by individual, but the general recovery pattern looks like this:
Day of Treatment – Day 1: Mild swelling with warmth may appear.
Days 2–3: Swelling is often most noticeable during this period.
Within 5–7 Days: Most swelling subsides naturally by this point.
As long as you get treated at least one week before your event, you'll be completely fine.
Swelling tends to be greater in the following cases:
A higher number of shots, and
those with thin or sensitive skin, or cases where higher energy levels were used,
or prominent areas such as around the cheekbones, which may take a bit longer to recover.
Many patients in the Hongdae–Hapjeong–Mapo area choose to get their treatment on Friday evening, recover over the weekend, and return to work on Monday.

3. Tips to Reduce Swelling Faster Before Important Events
There are key guidelines to follow in order to speed up your recovery after treatment.
Cold compresses for the first 48 hours after treatment are helpful,
and activities that rapidly raise your body temperature — such as saunas, jjimjilbang, or intense exercise —
can worsen swelling,
so it's best to avoid them for at least one week.
Alcohol and salty or heavily seasoned foods are also major culprits that increase swelling.
Staying well hydrated and slightly elevating your pillow while sleeping
helps promote lymphatic circulation.

Q1. I have an important event in 2 weeks — is it okay to get Ulthera + Thermage now?
A. In most cases,swelling subsides sufficiently within 7–10 days after treatment.
Two weeks is generally more than enough time, but results can vary depending on your skin condition and the scope of treatment,
so a pre-treatment consultation is the best way to determine the right timing for you.
Q2. After my Ulthera + Thermage treatment, one side of my face seems more swollen than the other — is that normal?
A. The left and right sides of the face have slightly different structures,
and lymph flow can also differ, which may cause swelling to appear asymmetrically.
In most cases, it evens out within 1–2 weeks.
However, if you experience severe pain or changes in skin color, it's best to come in for a check-up.
Q3. Can I massage my face to reduce swelling faster after Ulthera + Thermage?
A. Within the first week after treatment,please avoid strong massage or pressure.
Applying physical stimulation to tissue where thermal energy is still active can actually worsen inflammation.
Light lymphatic drainage strokes can be started carefully from 2 weeks post-treatment.
Swelling after Ulthera + Thermage typically resolves on its own within a week.
If you have an important event coming up, it's safest to get treated at least 2 weeks in advance,
and consulting with a specialist will help you find the ideal timing for your skin.

Q4. Ultherapy vs. Thermage — which does the doctor recommend?
For skin texture, Thermage is the way to go.
If facial contouring is a concern, I'd recommend Ulthera. 😄
Personally, I tend to recommend Ulthera quite often...
but for those over 40 with thinner skin, Thermage tends to get really positive feedback!
