Q. Does pelvic filler really
go on top of the bone?
A. Yes, that’s right.
Not directly under the skin, but beneath the fascia,
the rule is to inject into the space directly above the bone.
That way the volume doesn’t collapse and the line stays defined.
Q. Why does my pelvis look so angular?
A. Usually because above the greater trochanter
there is a hollow 'violin hip' structure.
It’s not something that gets solved by losing weight,
it’s a problem of bone and fascia structure.

They may look similar, but
the key difference is elsewhere
Pelvic filler is on the outer side of the hip,
specifically into the hollow area above the greater trochanter
by injecting filler to connect the line from the pelvis to the thigh
smoothly.
A hip lift is increasing the volume of the buttocks themselves,
while pelvic filler is the concept of 'filling in the dipped area.'
Unlike fat grafting
pelvic filler shows results immediately the same day,
and the biggest difference is that if you don’t want it
it can also be dissolved.

A part that is often misunderstood here
Director Wi Young-jin’s
key insight
Pelvic filler, injecting filler on top of the bone?
A dermatologist explains
the procedure principles and precautions, all in one place
The side of the pelvis looks sunken not because there isn’t enough fat.
To be honest, it’s a bone-structure issue.
It forms between the iliac crest and the greater trochanter of the femur
as a natural depression.
So to fill this space
not the superficial layer right under the skin,
but beneath the fascia and directly above the periosteum,
the filler must be placed on top of the bone.
Filler placed in the fat layer keeps getting compressed by movement
so it is absorbed faster,
meaning the injection depth determines how long it lasts.
Director Wi Young-jin’s key takeaway
Pelvic filler is not about filling with fat
but a procedure that fills the space above the bone.
Injection 'depth' determines how long it lasts.
No matter how much filler is used, if it’s injected too superficially
it won’t last 6 months.

This is how I classify it in my clinic
Type | Characteristics | Recommended approach |
Lean body type, depression type | Bones are prominent and the side line is angular | 15–20 cc per side, split into 2 sessions |
Standard body type, line type | There is fat, but the line doesn’t connect smoothly | 10–15 cc per side, finished in one session |
Postpartum deformity type | The pelvis widens and the sides sink in | Designed mainly to correct the greater trochanter area |
Filler is not permanent.
With HA filler, it usually lasts between 1 and 2 years
because it is gradually absorbed, so periodic retreatment is needed.
However, for people who need body contour correction or
find fat grafting burdensome,
it remains a practical option.
The three questions I get most often in the clinic
Q1. Can I exercise or
sit right after the procedure?
On the day, try to avoid positions that put prolonged pressure on the area.
It takes about 48 hours for the filler to settle.
I recommend avoiding gym workouts and yoga for about a week.
Q2. What about the cost and how long does it last?
Based on 10 cc per side,
the total for both sides is usually in the millions of won.
Duration is about 1 year at the shortest and around 2 years at the longest.
People who are very active tend to absorb it a bit faster.
Q3. What side effects or precautions should I know about?
Bruising, swelling, and asymmetry are the most common issues.
If too much volume is injected all at once
a lumpiness may remain,
so if the amount is large, I do it in two sessions.
Let’s continue with a deeper discussion in the next post.
