A heavy, droopy upper eyelid after glabella Botox usually shows up somewhere between day 3 and day 14. It lasts while the toxin is still acting on the muscle, then lifts on its own as the effect wears off, though it can take up to 12 weeks. Bruising or a mild headache is a different thing: that's a reaction at the injection site, and it's worth keeping separate from a droop.

If you're worried about this, you're not alone. Eyelid droop is one of the side effects people read about most before treating their '11 lines.' In this article, we'll cover why it happens, when it starts and fades, how to tell it apart from needle reactions, which signs mean you shouldn't wait, and how to plan the next session.

Quick Summary

  • A droopy eyelid after glabella Botox is uncommon, and it fades as the toxin wears off.

  • How often: in the approval trials, 13 of 405 people (3%) given 20 units in the frown area had it.

  • Bruising, swelling, and a mild headache on the day of treatment come from the injections themselves, so they're judged separately.

  • Get care right away: if swallowing, speaking, or breathing becomes hard, or you see double.

Why Can Glabella Botox Make an Eyelid Droop?

It happens when the toxin drifts far enough to weaken the muscle that lifts your upper eyelid. The vertical '11 lines' between your brows come from small muscles that pull the brows together when you frown, and those muscles sit close to the eyelid-lifting muscle just above the eye. If a little toxin spreads from the injection point and reaches it, the lid doesn't rise as far when you open your eyes.

*Levator muscle: the muscle that pulls the upper eyelid up so your eye opens.

In the mirror, a droopy eyelid looks different from a dropped brow. If your brows sit at the same height but the edge of the upper lid covers more of the colored part of your eye, that's eyelid droop (ptosis). If the brow itself has moved lower, the forehead muscle has lost some strength, and that has a different cause.

That's why placement matters. The injection points should stay more than 1 cm above the middle of the brow, and the points in the outer frown muscle should sit more than 1 cm above the bony rim around the eye*.

Before injecting, the doctor at our clinic first watches where your muscles bunch up when you frown and compares the height of your left and right brows, then decides the points and the dose. If one brow already sits higher than the other, or your eyelids tend to feel heavy, it's a good idea to record that difference in photos before treatment.

When Does a Droopy Eyelid Start, and How Long Does It Last?

A droop doesn't show up on the day of treatment. Botox starts relaxing the muscle 1 to 2 days after the injection and gets stronger over the first week. A droop follows that same curve, appearing on average between day 3 and day 14. As the toxin's effect fades, the muscle regains its strength and the eyelid lifts again.

Here's how common it is. In the approval trials for frown lines, 13 of 405 people (3%) who got 20 units of Botox developed a droopy eyelid, compared with none of the 130 people given a placebo. Keep in mind that these numbers apply to Botox (sold as Botox Cosmetic). Other brands, such as Xeomin, measure units differently, so the figure can't simply be carried over.

The weakness can last up to 12 weeks*. That's a ceiling, not a typical timeline, and how long it takes to lift differs from person to person.

Rarely, it lasts longer. In a case series of 7 people referred to eyelid specialists because their droop didn't go away, it lasted from 6 weeks to 13 months. That group was made up only of long-lasting cases, so it doesn't describe how long a droop lasts for most people who get one.

Bruising, Swelling, and Headaches: The Needle or the Toxin?

Bruising, swelling, redness, and tenderness at the injection points come from the needle rather than the toxin. Some people also get a mild headache about 2 to 4 hours after injections in the upper face, and an ordinary over-the-counter pain reliever usually handles it. These reactions show up on the day of treatment, while a droop appears days later, so timing alone helps you tell them apart.

A same-day headache isn't necessarily caused by the toxin, either. One study compared 202 people who got 20 units of Botox between the brows with 71 who got a placebo. Headaches were reported by 11% of the Botox group and 20% of the placebo group. Since the placebo was injected into the same spots with a needle, the injections alone can trigger a headache.

Here's a rough way to sort what you're feeling:

  • Day of treatment: bruising, swelling, redness, tenderness, or a mild headache. These come from the injection itself.

  • Day 3 to day 14: an eyelid that feels heavy or won't open as far. This is the toxin reaching a nearby muscle.

Glabella Botox Side Effects: What to Watch and When to Call

Bruising and swelling at the injection points, along with a mild headache on the day of treatment, are reactions you can watch at home. A heavy, droopy eyelid isn't an emergency either. Still, let the clinic that treated you know, and send a straight-on photo of your eyes. A research review found that eye drops that briefly lift the eyelid partly improved a droop, and whether to use them is decided at a visit after the doctor checks your eyes.

The signs below can appear when the toxin spreads beyond the injection area, so don't wait on them. They can show up anywhere from a few hours to a few weeks after treatment.

  • Trouble swallowing food or saliva: get medical care right away.

  • Slurred speech or a change in your voice: get medical care right away.

  • Difficulty breathing: go to the nearest emergency room right away.

  • Seeing double, or weakness throughout your body: get medical care right away.

  • An eyelid that covers your pupil so you can't see well: contact the clinic right away.

It's better to put off treatment if any of these apply to you:

  • You're pregnant or breastfeeding.

  • There's inflammation or an infection where the injections would go.

  • You've had an allergic reaction to a botulinum toxin product before.

  • You have a condition that weakens muscles, such as myasthenia gravis.

Planning Your Next Glabella Botox After a Droop

Book the next session only after the droop has fully lifted. If you're injected again while some droop remains, it becomes hard to tell a new droop from the old one. For the next round, the doctor looks at which eye was affected last time and how many days after the injection it started, then moves the points farther from the brow or lowers the dose.

Taking a straight-on photo of your open eyes on the day you first notice the droop, and then once a week in the same light, makes it easier to judge for yourself how quickly it's lifting. Those photos also give the doctor something concrete to compare at your next visit.

Keep a simple record of the treatment itself, too:

  • The date of the injection

  • The product name

  • Which eye drooped, and on which day after treatment

Wait Until It Lifts, Then Re-Plan

Fitting Glabella Botox Into a Trip to Korea

Unless you've had sedation that day, there's no reason to postpone your flight home after glabella Botox.

It helps to know what one visit can do. Some treatments, such as fillers, change your shape the same day. Most others don't change things dramatically at once; they bring you gradually closer to how you used to look. With Botox, the effect builds over the first week, and if a droop appears, it usually shows between day 3 and day 14, which may be after you've already flown home.

Once you're home, you can send photos through the consultation button on this page if something doesn't look right. If you notice any of the warning signs above, such as trouble swallowing or breathing, see a local doctor or emergency room first rather than waiting for a reply.

The Bottom Line: Ask Where the Points Will Go

Whether a droop happens has a lot to do with how far from your brow the points are placed and how much goes into each one. So before you decide, ask: "For my frown lines, how far above the brow will the points go, and how many are there?" That one answer makes it easy to compare with what was done last time. If side effects are your main concern, talk to your provider before deciding.

Beautystone is a clinic in Seoul's Hapjeong area, and you can reach us through the consultation button on this page. In your first message, include the date of your last injection, the product name, and which eye drooped, if any did.
