Men's Forehead Filler: Hollow or Just Lines?

Forehead filler fits men whose forehead looks flat, or hollow in the center or at the sides, even with a relaxed face. If your main concern is horizontal lines that show up only when you raise your brows, or sagging, filler isn't aimed at the right problem.

The forehead is also one of the areas most often linked to blocked blood vessels after filler, so it's worth knowing the warning signs before a first treatment. Below, we go through how to tell a volume problem from a wrinkle problem, what studies show about amounts and longevity, and what to watch for afterward.

At a Glance

  • Good fit: a forehead that looks hollow or flat even when your face is at rest.

  • How long it lasts: hyaluronic acid filler usually lasts 4 to 12 months; in one study using a deep layer, the shape held for up to 36 months.

  • Redness, swelling, and bruising often settle within 7 to 14 days.

  • Skin turning white or blue, or unusual pain, means getting medical care right away.

Is Your Forehead a Filler Case or a Wrinkle Case?

Relax your face, then look at your forehead from the side in a mirror. If the center still looks dented, or the sides near your temples look sunken so your forehead seems narrow and flat, you're looking at a volume problem. Forehead hollows are often grouped into central, side, mixed, and whole-forehead types, and the type decides where filler goes and how much.

Now raise your eyebrows. If the horizontal lines deepen only when you do that and fade once you relax, they come from the movement of the forehead muscle. That's territory for Botox, which reduces muscle movement, rather than filler. A forehead and brow that have dropped together is a separate concern from lost volume as well, and it's looked at on its own terms.

Plenty of men have a bit of both, so the mirror check is a starting point, not a verdict. It simply tells you which question to bring to the consultation: "Is this hollow?" or "Is this movement?"

Forehead shape is also seen together with your brows, nose, and chin. Before choosing where filler goes, the doctor looks at the balance of your whole face and any difference in height between the left and right sides of your forehead.

How Much Filler Do Men Get, and How Long Does It Last?

Hyaluronic acid filler usually lasts 4 to 12 months, and more can be added once the area starts to look hollow again. In a study of 38 people, 36 men and 2 women, an average of 16 mL was placed in a deep layer right on top of the bone, and the shape held for as long as 36 months.

That study looked back at records from a single clinic with no comparison group, and the amounts were meant to reshape the entire forehead contour. For a first treatment, starting with only as much as the hollow actually needs tends to look more natural. Rather than filling everything at once, it's better to let the swelling go down, look at the shape, and then decide whether to add more.

How long filler lasts depends on the layer, the amount, and the type of filler, so the study numbers aren't a promise for your forehead. Results differ from person to person.

Why Does the Forehead Need Extra Care With Filler?

Blood vessels that come up from around the eyes run through the forehead, and if filler blocks one of them, it can lead to skin damage or problems with vision. A study pooling 290,307 hyaluronic acid filler treatments from multiple clinics in Japan found 10 cases of blocked vessels, and 8 of those were in the forehead.

That's very rare, but the consequences are serious. A review of 365 cases of vision loss after filler, reported between 2018 and 2023, found the forehead was the second most common site at 27.7%, after the nose. Of the 318 cases with a recorded outcome, 68.2% didn't regain their vision.

That's why forehead filler is often placed beneath the muscle, close to the bone, to stay away from the shallower layers where vessels are more common. At your consultation, ask which layer will be used and whether a medicine that dissolves filler can be used right away if a problem comes up.

Filler is a medical procedure, so don't get it at a salon, at home, or anywhere that isn't a medical facility. When it's done in those settings, reports of lumps, skin discoloration, infection, and severe swelling rise sharply.

Forehead Filler Side Effects: What Fades and What's Urgent?

Redness, swelling, tenderness, and bruising at the injection sites are common and often settle within 7 to 14 days. Hold off on strenuous exercise until the next day, and try not to touch your forehead for 3 days.

Usually fades on its own

  • Swelling, and a sore feeling when you press on your forehead

  • A small bruise at an injection site that gradually fades

Get medical care right away if you notice

  • Skin near an injection site turning white, gray, or blue

  • Unusual pain

  • A net-like red or purple pattern on the skin, or blisters appearing days later

  • Changes in your vision, such as blurring or loss of sight

The US FDA lists these as stroke warning signs: sudden trouble speaking, numbness or weakness in the face, arms, or legs, difficulty walking, vision changes, face drooping, severe headache, dizziness, and confusion. If your vision changes or any of these signs appear, go to the nearest emergency room. For skin color changes or unusual pain, get seen by a doctor immediately. A message through the consultation button on this page can be added on top of that visit, but it doesn't replace it.

What Should You Mention Before Your First Forehead Filler?

A few conditions need to be shared before treatment, and if your forehead skin is inflamed or infected that day, the treatment is postponed. Tell the doctor if any of these apply:

  • Inflammation or infection on your forehead

  • A condition that makes bleeding hard to stop

  • Severe allergies or anaphylaxis in the past

  • An allergy to lidocaine or similar numbing medicines

  • Blood thinners such as aspirin or warfarin, or supplements like ginseng or vitamin E

It also helps to say whether you've had any earlier treatment on your forehead, and what it was. Bring the product name if you know it.

Forehead Filler on a Korea Trip: Flying, One Visit, and Home

Unless you were sedated that day, there's no reason to delay your flight after forehead filler. The injections themselves don't keep you from flying.

Filler is one of the treatments where the shape changes on the same day. Swelling can make the area look fuller at first, so the result is easier to judge once it has gone down. Neither the size of the change nor how long it lasts is something a single visit can promise.

Once you're home, you can send photos through the clinic's consultation channel if something seems off. If your skin turns white or blue, you have unusual pain, or your vision changes, go to a local hospital or emergency room first rather than waiting for a reply.

The Bottom Line: Ask About the Layer Before the Amount

The question that matters most at a first consultation is which layer the filler goes into, how far it extends, and how many mL you'd start with. Ask those three in that order, and comparing your options gets a lot easier. Before that, settle one thing for yourself: does your forehead look hollow even at rest, or do the lines show up only when you move?

Beautystone is a clinic in Seoul's Hapjeong area, and you can reach out through the consultation button on this page. In your first message, include relaxed front and side photos of your forehead, whether you've had forehead treatments before, and any medications you take.
