Dark circles: Why no treatment seemed to work
Dark circles: Why no treatment seemed to work
Dark circles won't fade? Learn why type matters—pigment, vascular, or structural. Find your fix.
Why No Treatment Has Worked for Your Dark Circles

As the temperature gap between day and night widens,
many patients coming into the clinic these days
look overall more dull and fatigued.
It's also a season when skin tone itself becomes unstable,
and if you lose a little sleep or get stressed,
the under-eye area is always the first to show it.
"I've tried dark circle creams
and even laser treatments — so why am I not seeing any results?"
This is something
I hear constantly in the clinic.
But the truth is,
if you get treated without identifying the root cause,
it's only natural that nothing works.
Dark circles aren't just one thing.
That's what I want to talk about today.
💡 Key Takeaways from Today's Post
Q. Why does dark circle treatment keep failing?
A. If treatment is done
without identifying whether the cause is pigmentation, vascular, or structural,
it simply won't work.
Q. How can you tell the different causes apart?
A. You can roughly distinguish by
color (brown / purple / blue), whether it lightens when the skin is pulled,
and whether there is any bulging in the area.
Why Dark Circles Form
— The Three Causes Are Completely Different

This is something many people get wrong:
dark circles are not a single condition.
Even though the result — under-eyes that look dark — is the same,
the causes fall into three distinct categories.
1. Pigmentation Type
This occurs when melanin pigment
becomes hyperpigmented in the under-eye skin.
UV exposure, friction, and chronic rubbing
from atopic dermatitis or allergic rhinitis are common causes.
It often appears brownish,
and when you gently pull the skin with your finger,
if the color moves along with it,
it's likely the pigmentation type.
2. Vascular Type
This occurs when the under-eye skin is thin
and the blood vessels beneath show through.
It often appears bluish or purplish,
and if the color looks even more distinct when the skin is pulled,
it's closer to the vascular type.
3. Structural Type (Shadowing)
This occurs when under-eye fat protrudes forward,
or when cheek volume deflates,
making the under-eye area look sunken.
This isn't a pigment or vascular issue —
it's a matter of three-dimensional contour.
The shadow itself is what creates the dark appearance.
👨⚕️ Dr. Wi Youngjin's Clinical Note:
In my experience, more than half
of the patients I consult are mixed type.
They have pigmentation and structural hollowing at the same time.
In these cases, laser alone isn't enough —
volume restoration needs to be combined for overall improvement.
That's why, when only one treatment is done,
it often feels like only half the problem was addressed.
Your Dark Circle Type
— How to Check at Home
Method 1 — Gently Pull the Skin
Lightly pull the under-eye skin to the side.
Color lightens → possible pigmentation type
Color becomes more distinct → possible vascular type
Method 2 — Change the Lighting Direction
When light is shone from above,
if your under-eye area looks darker,
there's a good chance it's structural type (shadow type).
Targeted Treatment by Cause Type

● Pigmentation-Type Dark Circle Treatment
The standard approach is laser targeting melanin.
Because under-eye skin is thin,
energy levels need to be set conservatively.
For this area, I typically
divide treatment into multiple sessions at short intervals
and take a gradual approach.
● Vascular-Type Dark Circle Treatment
A laser that directly targets
blood vessels is required.
Combined with this, a skin booster like Rejuran
to increase skin thickness should be used alongside
to maintain results.
● Structural-Type Dark Circle Treatment
This cannot be resolved with laser.
The hollow under-eye volume needs to be filled
in order to eliminate the shadow.
Precise filler injection into the tear trough
is the most commonly used method.
⚠️ Things to Check Before Treatment
1. Precautions for Filler Procedures
The under-eye area has many blood vessels and thin skin,
so if done incorrectly, it may appear swollen
or cause a bluish discoloration known as the Tyndall effect.
2. Precautions for Laser Procedures
Energy must be used conservatively.
If the approach is too aggressive,
it can actually worsen post-inflammatory hyperpigmentation (PIH).
3. Lifestyle Management Alongside Treatment Is Essential
If you have allergic rhinitis,
reducing rubbing around the eyes should come first.
Lack of sleep also worsens vascular-type dark circles,
so managing sleep quality alongside treatment is important.
