What is filler firmness?

The firmness of a filler is often represented by a value called G'*. It refers to how well the filler resists pressure, meaning how well it holds its shape when pressed or moved. If it is highly cross-linked, it has a high G' value; if it has less cross-linking, the value is lower.

* G' (G-prime, storage modulus): A rheological value representing the elasticity of the filler. The higher it is, the firmer it is and the better it holds its shape, making it ideal for contouring areas. The lower it is, the softer and more spreadable it is, making it perfect for fine wrinkles and lips.

The reason even the same brand has separate lines for lips, nasolabial folds, and the chin is that they developed them with different G' values. That is why instead of just looking at the overarching name "Juvederm," it is important to know which specific "Juvederm line" is being used.

Soft vs. firm texture: Where are they used?

Soft fillers are injected into the superficial layers. The HA* (hyaluronic acid) filler is gently placed close to the dermis to smoothly fill fine wrinkles or naturally shape the lips. If you fill the lips with a firm filler, the margin can look unnatural or be felt under the skin. For lips or fine lines around the eyes, a soft texture is the perfect match.

* HA (Hyaluronic Acid): A polysaccharide substance naturally present in our skin. It has a high capacity to retain moisture, so when formulated into a filler, it provides both hydrating and volumizing effects.

Firm fillers go into the deeper layers. For areas that require structural support, such as the chin, nose tip, and front cheekbones, a firm filler is used to support the structure close to the subcutaneous and periosteum layers. If you place a soft filler here, even in large amounts, the structural holding power is weak, and the shape can easily collapse. In Korean and Asian clinical cases, filling the contours with a firm texture is often the ideal solution — as the goal is usually to naturally minimize visual empty spaces on the face.

Depth goes hand-in-hand with firmness

Firmness and injection depth go together as a pair. If you place a firm filler in a superficial layer, nodules and lumps can be felt, whereas injecting a soft filler deep down requires too much volume to achieve any visible results. They are generally matched like this:

  • Superficial Dermis → Soft Texture (Lines like Soft, Volbella, Light)

  • Mid Dermis → Medium Texture (Lines like Volift, Balance, Define)

  • Subcutaneous · Periosteum → Firm Texture (Lines like Volux, Lyft, Atiere Volume)

This is why even for the same person, different levels of firmness are used depending on the treatment area. Even for the exact same area, the choice may vary depending on the individual's skin thickness and texture.

Even at the same level of firmness, each brand utilizes different cross-linking technologies. Juvederm uses VYCROSS/HYLACROSS, Restylane uses OBT/NASHA, and Belotero uses CPM. Therefore, even within the same soft texture category, the level of integration and duration may differ minutely. A client's first brand choice is decided by collectively looking at their skin texture, past treatment history, and budget.

Q. Is a firmer filler always better?

It depends entirely on the area. For areas that need defined structure, like the chin and nose, a firm texture looks most natural. However, for superficial layers like the lips and eye area, a soft texture is much more suitable. It is not about "firmer is better," but rather using the right product in the right place.

Q. Can I receive multiple brands in a single session?

Yes, absolutely. Many clients receive fillers of different firmness levels in different facial areas on the same day. For example, we can gently shape the lips with a soft filler while defining the chin with a firm filler. We will carefully decide which firmness level suits each of your facial areas during your consultation.

Q. If two brands have the same firmness for the same area, what is the difference?

Due to different cross-linking technologies, there are subtle differences in how the gel integrates into the skin and how long it lasts. Some brands excel in blending seamlessly with natural tissues, while others are great at providing sharp, defined support. We will explore each of these characteristics in detail in the next chapters of our series.
