You might visit a clinic to have a tiny mole removed, only to find a faint mark still remaining in the exact same spot six months later. It is easy to think, "It is just a mole removal, so it will disappear cleanly," but it is actually quite common for the color to take a long time to fade, or for the area to just look slightly different.
To give you the bottom line, marks left after mole removal do not mean the treatment failed. Rather, it is determined by the combination of the mole's depth and the treatment method. Surface moles tend to disappear cleanly in one go. However, for deep moles with pigment reaching into the deep dermis, removing them all at once can easily leave scars. Because of this, we intentionally split the process into multiple sessions. During this journey, it is completely natural for a faint mark to remain visible for a certain period.
Understanding How Deep the Mole Reaches First
The appearance of a mole varies depending on which layer of the skin—the epidermis, dermis, or deep dermis—the pigment cells are clustered in. Moles with pigment located only in the epidermal layer tend to be flat and look like a light brown wash. Moles that have reached down into the dermal layer look slightly raised to the touch or have a dark, intense black color.
Epidermal moles can often be removed in a single treatment by gently shaving away the pigment. On the other hand, for moles that go deep into the dermis, reaching that depth all at once can easily leave scars. Therefore, we intentionally divide the process into two or three sessions, shaving them off layer by layer from the surface. Consequently, even for what seems like a similar mole, differing depths will lead to different session counts and recovery timelines.

The CO2 Laser Shaving Method
This is the most common method for mole removal. We use a CO2 laser to finely shave the mole site, removing the surface pigment. Epidermal moles are often removed in just one session, and the procedure itself is completed within 1 to 2 minutes.
After the treatment, a small scab will form on the area and naturally fall off in about 7 to 10 days. Once the scab falls off, the skin will look pinkish before gradually blending with your surrounding skin tone over 3 to 6 months. Whether this pinkish mark turns into PIH (Post-Inflammatory Hyperpigmentation) or a scar becomes clearer over time. If you do not take proper care with UV protection, PIH commonly lingers for a long period.
PIH (Post-Inflammatory Hyperpigmentation)*: A phenomenon where more melanin is deposited on the area after the skin is stimulated. It occurs more easily on darker skin tones and lasts longer if daily UV protection is neglected.

Why Deep Moles are Removed in Multiple Sessions
If a mole extending deep into the dermis is shaved down to that depth all at once, it causes significant damage to the dermis. When the dermis is heavily damaged in one go, collagen can form in a disorganized structure, making it highly likely to leave a sunken scar. This is why we split the process into two or three sessions, shaving just one layer from the surface at a time, allowing the dermis time to heal in between.
Although this approach takes more time, it carries a much lower risk of scarring. You might want to get it over with in a single session, but if the doctor advises during your consultation that "it is better to remove this mole in about two separate sessions," it is highly likely a choice made to minimize scarring. The interval between sessions is usually set at 3 to 6 months.

Other Options Depending on Treatment Methods
Faint moles or blemishes located only in the epidermal layer can also be lightened using toning (Pico, Nd:YAG) or IPL instead of CO2. Since this method breaks down the pigment without shaving the skin, allowing it to fade cumulatively, its key advantage is that it leaves almost no surface marks. However, they do not disappear in one go; the change becomes distinct only after multiple cumulative sessions spaced 4 to 6 weeks apart.
If you have multiple moles clustered in an area about the size of a palm, you might also consider punch excision, where the area is punch-excised and sutured. While punch excision offers fast results, it leaves a small linear scar. On the other hand, toning has a lower risk of scarring but takes longer. Since the best method varies depending on the depth, number, and location of the moles, it is safest to decide together during your consultation.
