Xeomin vs. Regular Botox: What's the Difference?

Xeomin vs. Regular Botox: What's the Difference?

Xeomin vs. Regular Botox: What's the Difference?

Xeomin vs. Regular Botox: Lower Resistance Risk?

Perhaps because the weather is so lovely during this change of seasons,

I've recently found myself frowning and furrowing my brows.

I guess it's about time for me to get a treatment too. Haha.

Once you start noticing frown lines,

Botox naturally comes to mind.

However, when you actually start looking into it, the sheer variety of options can be quite confusing.
Many people aren't sure of the difference between Xeomin and Botox,
so they often just go with the name they recognize.

Today, I will lay out those differences clearly for you.

Q. Is Xeomin a completely different drug from Botox?
A. No, they belong to the same family (Botulinum Toxin), but their formulation molecular structure is different.
Coretox and Xeomin have a molecular weight of around 150 kDa, whereas others are around 900 kDa.

Being about 1/6th lighter, this formulation helps reduce the risk of developing resistance.

Q. Is Xeomin better for frown lines?

A. It varies from person to person, but it can be highly beneficial for those who have already developed resistance to other Botulinum products.

Q. Does Xeomin work faster than regular Botox?

A. In my experience, it does tend to show effects slightly faster.

First, let's understand what Xeomin is

There are several types of botulinum toxin products available.
The most commonly used ones in Korea include
Botox (Allergan), Dysport, Neuronox, and Xeomin.

Among these, Xeomin has one crucial difference compared to other products.

It is a 'naked toxin', meaning it contains no complexing proteins.

Standard botulinum products have
complexing proteins attached surrounding the neurotoxin.
However, Xeomin is a highly purified toxin from which these unnecessary proteins have been removed.

According to the US FDA Botulinum Toxin Information,
Xeomin is a product that received FDA approval for the treatment of frown lines (glabellar lines) in 2011.

Why is this important?

Because without auxiliary complexing proteins, the likelihood of your body forming antibodies is significantly reduced.

In other words, this is precisely why Xeomin is an excellent alternative for patients who have been receiving Botox for a long time and feel it is no longer as effective as before.

Why do we use Xeomin on frown lines?

Frown lines mainly involve two muscles:
The corrugator supercilii (the muscle that pulls the eyebrows in and down) and
the procerus (the muscle that runs down the bridge of the nose).

Overactivity in these two muscles
creates those vertical "11" lines,
and botulinum toxin temporarily relaxes their movement.

But there is one vital point to keep in mind here.

More than which product you use, how precisely and where it is injected is what truly matters.

While there are formulation differences,
even a minor error in injection placement
can lead to side effects like heavy, drooping eyebrows or awkward, unnatural facial expressions.

Clinical reports suggest that Xeomin has a slightly more localized diffusion range,
which can actually work to your advantage in delicate areas like the glabella.

Dr. Wi Young-jin’s Practice Notes:

Patients trying Xeomin for the first time often tell me,
"It worked much faster than I expected."
In my experience, the onset of action takes about 3 to 4 days, which feels about 1 to 2 days faster than regular Botox.

The downside is the cost ^^...
It has a higher unit cost than regular Botox,
and it is not carried by every single clinic.

Even so, for those who feel their usual treatments are losing their touch after multiple sessions,
or for those who want to prevent developing resistance right from their very first treatment,
it is an option highly worth considering.
I always make sure to discuss this point with my patients beforehand.

Let's compare Xeomin vs Regular Botox directly

A common misconception is that
Xeomin is always the superior choice.

Your choice should vary depending on your specific situation.
I have summarized this in the table below.

Feature

Xeomin

Regular Botox

Complexing Proteins

None (naked toxin)

Present

Antibody Formation Risk

Theoretically low

Relatively higher

Onset of Effect

Slightly faster in my experience

Usually 4 to 7 days

Diffusion Range

Limited (advantaged for frown lines)

Relatively broader

Duration

3 to 4 months (varies by individual)

3 to 4 months (varies by individual)

Cost

Relatively higher

Relatively lower

Storage Conditions

Can be stored at room temperature

Requires refrigeration

Key Summary: Xeomin is a pure botulinum toxin free of complexing proteins, making it advantageous for those with built-up resistance or for precise areas like frown lines, though it does carry a higher cost.

I highly recommend Xeomin to the following individuals

To be perfectly honest,
I do not necessarily recommend Xeomin as the first choice for beginners.

While it depends on the individual case, this is how I usually advise my patients:

  1. Those who have been getting Botox consistently for over 3 years and feel the duration of the effect has shortened

  2. Those who need overall dosage management because they are treating multiple areas at once, such as the forehead, eyes, and glabella

  3. Those who want to minimize the possibility of antibody formation from their very first treatment

For these individuals, Xeomin is a choice with clear, practical benefits.

On the other hand, if you are receiving treatment for the first time
or if the cost is a significant concern,
regular Botox can still provide you with wonderfully satisfying results.

Even during WhatsApp consultations prior to your visit, Dr. Wi Young-jin provides honest advice
on which formulation is truly the best fit for your unique situation.

Frequently Asked Questions

Q1. How long do the effects of Xeomin for frown lines last?

A. In my experience, it usually lasts about 3 to 4 months. The duration may be slightly shorter for those with stronger muscle mass or highly active facial expressions. The overall longevity does not differ significantly from regular Botox.

Q2. Regular Botox didn't work well for me. Will Xeomin be different?

A. We first need to pinpoint why it wasn't effective. If it was due to antibody formation, Xeomin could certainly be of help. However, if the issue stemmed from injection placement or incorrect dosage, it is better to reassess the technique rather than the product type. We can determine this during your consultation.

Q3. Are there any specific post-treatment precautions for Xeomin?

A. They are exactly the same as regular Botox. Avoid lying down for 4 hours after the procedure, refrain from strenuous exercise or drinking alcohol on the day of treatment, and do not massage the treated area. Following these simple steps is all you need.
